We agree that the problems are endogenous to a system, however this does not mean that the causes are purely endogenous or exogenous.

A problem situation will combine a number of causes (endogenous and exogenous), and is somewhat similar to a disease, the immune system is endogenous (a person) and a drop of it, would allow certain viruses enter, and to generate a disease.

The problems are the result of a chain or combination of factors, at a time can not be resolved automatically and require an intervention to order the factors involved in the chain of causes that originated it. Hope this helps somewhat.
