Martin, thanks for your excellent analysis of school 'systems'.
Based on my experiences teaching both 4th grade, 46 students, and 2 different special-ed high schools, I would add only 1 more item, but a critical one. Where in your hierarchy it fits, I'm not sure, but very close to your #1.
Real students are Individuals! They have widely varying needs, strengths, weaknesses, and learning styles. Not addressing these issues can often make the transfer of knowledge fail.
